PHQ-9 Depression Test

This Questionnaire is a brief, nine-item questionnaire used to screen for and assess the severity of depression. It's a tool to identify individuals who may be experiencing depression and to track changes in symptoms over time.

1. Over the last 2 weeks, have you had little interest or pleasure in doing things you usually enjoy?

2. Over the last 2 weeks, have you felt down, depressed, or hopeless?

3. Over the last 2 weeks, have you had trouble falling asleep, staying asleep, or have you been sleeping too much?

4. Over the last 2 weeks, have you felt tired or had very little energy?

5. Over the last 2 weeks, have you had a poor appetite or found yourself overeating?

6. Over the last 2 weeks, have you felt bad about yourself — or felt like you are a failure or have let yourself or your family down?

7. Over the last 2 weeks, have you had trouble concentrating on things, such as reading or watching television?

8. Over the last 2 weeks, have you been moving or speaking so slowly that other people noticed? Or, have you been so restless or fidgety that you were moving around much more than usual?

9. Over the last 2 weeks, have you had thoughts that you would be better off dead or thoughts of hurting yourself in some way?

This is calculated by assigning scores of 0, 1, 2, and 3 to the response categories, respectively, of "Not at all," "Several days," "More than half the days," and "Nearly every day." The total score for the nine items ranges from 0 to 27.

  • 0-4: None-minimal depression
  • 5-9: Mild depression
  • 10-14: Moderate depression
  • 15-19: Moderately severe depression
  • 20-27: Severe depression
