Anxiety Test

The Anxiety Test is a brief self-report questionnaire used to screen for anxiety state and assess the severity of its symptoms. It consists of seven questions, each asking about how often a person has experienced certain anxiety symptoms in the past two weeks, with responses on a 4-point Likert scale.

1. Over the last 2 weeks, have you felt nervous, anxious, or on edge?

2. Over the last 2 weeks, have you had difficulty stopping or controlling your worrying?

3. Over the last 2 weeks, have you found yourself worrying too much about different things?

4. Over the last 2 weeks, have you had trouble relaxing?

5. Over the last 2 weeks, have you felt so restless that it was hard to sit still?

6. Over the last 2 weeks, have you become easily annoyed or irritable?

7. Over the last 2 weeks, have you felt afraid as though something awful might happen?

If you checked any problems, how difficult have they made it for you to do your work, take care of things at home, or get along with other people?

This is calculated by assigning scores of 0, 1, 2, and 3 to the response categories, respectively, of “not at all,” “several days,” “more than half the days,” and “nearly every day.” GAD-7 total score for the seven items ranges from 0 to 21.

  • 0–4: minimal anxiety
  • 5–9: mild anxiety
  • 10–14: moderate anxiety
  • 15–21: severe anxiety
